HRSA PCA Cooperative Agreement
The HRSA Primary Care Association (PCA) Cooperative Agreement funds PCAs to support health centers in the vision of providing comprehensive, high quality primary health care and improving the health of individuals and communities. DCPCA provides coordinated training, technical assistance, and the facilitation of learning collaboratives through the combined efforts of our quality improvement, policy, and health information and technology services. Increase access to comprehensive primary care.
Provides strategic planning trainings for board and leadership on financial sustainability, innovation, and leadership.
Supports health centers in expanding patient access to integrated behavioral health, SUD services, and high-quality maternal health services.
Accelerate value-based care delivery.
Supports implementation of effective tools for population health management and clinical and operational innovations, leading health information technology and exchange systems in the District.
Addresses strategic challenges to value-based care via trainings oriented to executives and providers.
Foster a workforce to address current and emerging needs.
Supports development of workforce plans for health centers and dissemination of best practices for recruitment and retention.
Strengthens the integrated behavioral health model, improving the pipeline for behavioral health staff, and implementing trauma-informed care processes for workforce success.
Facilitates expansion of innovative multidisciplinary team models and promotes growth of quality improvement infrastructure to support innovation in an evolving landscape.
Offers executive support, including trainings and policy support to improve the financial management and revenue cycle for health centers.
Enhance emergency preparedness and response.
Convenes the Emergency Preparedness Task Force to share best practices and address challenges among health center staff for long-term preparedness and short-term response to emergencies.
Supports health centers in improving the utility and comprehensiveness of emergency plans.
Conducts customized trainings on priority topics, including pandemic response and active threat.
Improves integration between health centers and District-wide stakeholders and response plans.
Advance health center clinical quality and performance.
Facilitates health center access to the people, tools, and information needed to enhance clinical quality and performance.
Provides training and technical assistance to health centers to engage in continuous quality improvement of care for diabetes, hypertension, and cancer screening.
Leads DC PACT, a coalition of over 50 DC-based to address the immediate health-related social needs of our community.
Supports health centers in advancing their ability to collect, share, and utilize data on social risk factors and social determinants of health (SDOH) for improved patient care.
